Direct Patient Care

Her Health offers direct patient care which provides healthcare on your terms. Memberships allow your provider to work directly for you, the patient, and help to remove the barriers to care that insurance companies have placed on patients and providers. Overall, this allows patients to be more satisfied with their care.

What is included in my membership?

Your monthly subscription includes direct access to your dedicated nurse practitioner, same or next day appointments, telehealth visits available 5 days per week, 6 primary care visits and 1 women's wellness exam with pap smear included per year. Additionally, you will receive medications and lab tests at the providers cost which are not included in the membership.

What is NOT included in my membership?

Memberships do not include medications, diagnostic blood tests or imaging, long term contraception devices, or additional visits outside of the 6 primary care and 1 women's wellness exam per year. Services not provided include chronic pain management, DOT physicals, disability determination, or hospital admission/discharge.

What is the cost of a direct patient care membership?

The cost of a direct patient care membership is $150 per month. A $100 registration fee is required which is due at the time of sign up. Membership fees are due on the 1st of each month or can be paid 6 months or 1 year in advance. Memberships are not a replacement for health insurance. You may choose to pair your membership with a high deductible or cost-sharing policy. Many patients choose to pair their coverage in this way to allow them the individualized care they prefer, while being prepared for life's unexpected events. Cancellations can be made with a 30 day written notice. If requesting re-enrollment, the $100 registration fee and current membership pricing is due at the time of registration. There is no guarantee for re-enrollment after a cancellation has been made.

What if I already have insurance?

If you currently have an insurance policy in place, we will gladly file your visit with your insurance carrier on your behalf. You will still enjoy the many benefits of your membership including direct access to your dedicated nurse practitioner, same or next day appointments, telehealth visits 5 days per week, and discounted blood tests and medications, including long-term contraception devices and BOTOX®.
